Immune escape and immune camouflage may reduce the efficacy of RTS,S vaccine in Malawi

Abstract: The RTS,S/AS01 malaria vaccine will undergo a pilot vaccination study in sub-Saharan Africa beginning in 2019. RTS,S/AS01 Phase III trials reported an efficacy of 28.3% (children 5-17 months) and 18.3% (infants 6-12 weeks), with substantial variability across study sites. We postulated that the relatively low efficacy of the RTS,S vaccine and variability across sites may be due to lack of T-cell epitopes in the vaccine antigen, and due to the HLA distribution of the vaccinated population, and/or due to 'immune… Show more

“…Optimal protection from immunisation requires a four-dose schedule. 12,19 But the results suggest that the long-sought after vaccines are possible. Indeed, about 20 vaccines based on whole organisms or well-characterised antigens are in clinical trials.…”

“…This homology analysis is performed using the JanusMatrix tool, which examines human sequence similarity with respect to the HLA and T cell receptor (TCR) faces of an epitope to flag sequences that could potentially elicit undesired autoimmune or Treg responses due to homology with sequences encoded by the human genome (32). The JanusMatrix (human homology) score of a given amino acid sequence indicates the number of potential immunosuppressive response triggers or flags, with higher JanusMatrix scores indicating a bias towards immune tolerance (33). Note that additional information concerning the role of the Treg repertoire in maintenance of self-tolerance can be found in Feng et al (34).…”

The JanusMatrix (human homology) score of a given amino acid sequence indicates the number of potential immunosuppressive response triggers or flags, with higher JanusMatrix scores indicating a bias towards immune tolerance (32). Ninety-five percent of randomly generated predicted ligands to HLA-DRB1 supertype alleles have JanusMatrix (human homology) scores between zero and two.…”
