Immune‐related hepatitis related to checkpoint inhibitors: Clinical and prognostic factors

Abstract: Background & Aims: Check point inhibitors (CPI) have improved survival of oncology patients but adverse effects that mimic autoimmune disorders have been reported. Our aim was describe the characteristics of immune-related hepatitis (irH) and prognosis, and compared them to those of patients with autoimmune hepatitis (AIH). Methods: This is a retrospective study including all grade ≥ 3 (severe) irH diagnosed among 414 patients treated with CPI from 2016 to 2018. Results: Twenty-eight cases of severe irH were r… Show more

“…Patients with liver toxicity on ICIs are usually asymptomatic and the symptoms, when present, are particularly non-specific; they may present with fever, skin rash and, in rare cases, jaundice. 66,67 Symptoms deriving from concomitant toxicity affecting another organ, such as colitis, hypophysitis or pneumonitis, may also occur. 68 No male or female preponderance has been described and age does not appear to constitute a risk factor for the development of liver toxicity.…”

“…68 No male or female preponderance has been described and age does not appear to constitute a risk factor for the development of liver toxicity. 66,67 The interval elapsing between the initiation of therapy and the onset of liver toxicity varies considerably, and toxicity may even occur after treatment discontinuation. In patients treated with ipilimumab as adjuvant therapy for stage III melanoma, the median time to the onset of hepatic irAE was 8.9 weeks (ranging from 1.9 weeks to 145.4 weeks).…”

“…Termed immune related adverse events (irAEs), the side effects of these novel therapeutic antibodies result from the loss of immune homeostasis and off-target effects in peripheral tissues [2,3]. Although the skin, endocrine glands and digestive tract are mostly affected, pulmonary [4][5][6], neurologic [7][8][9][10], hepatic [11][12][13] and cardiologic [14][15][16][17][18][19] side effects have also been described and may be life-threatening.…”
