Immunogenetics of autoimmune thyroid diseases: A comprehensive review

Abstract: Both environmental and genetic triggers factor into the etiology of autoimmune thyroid disease (AITD), including Graves' disease (GD) and Hashimoto's thyroiditis (HT). Although the exact pathogenesis and causative interaction between environment and genes are unknown, GD and HT share similar immune-mediated mechanisms of disease. They both are characterized by the production of thyroid autoantibodies and by thyroidal lymphocytic infiltration, despite being clinically distinct entities with thyrotoxicosis in GD… Show more

“…There are well-established associations between alleles of the major histocompatibility complex with GD, with susceptibility being carried with HLA-DR3 and HLA-DR4 haplotypes [13]. Other susceptibility loci at which association has been replicated include those at cytotoxic T lymphocyte antigen-4, protein tyrosine phosphatase nonreceptor-22, basic leucine zipper transcription factor 2, and CD40 [14]. A noncoding variant within the TSH-R gene itself also confers susceptibility.…”

“…Не вызывает сомнения, что больные нуждаются в иммунотроп-ной терапии. При выборе адекватного иммуномо-дулятора для таких больных необходимо учитывать способность препарата влиять на указанные пара-метры иммунограммы [12][13][14][15]. Наш собственный опыт касается сравнительной оценки влияния на иммунограмму при аутоиммунной тиреопатологии иммуномодулятора эрбисола и селенсодержащего препарата цефасель [10].…”

“…The reported associations for GD were statistically significant, but with much smaller OR around 2–3 for previous live births. Both autoimmune hypothyroidism and GD are characterised by intrathyroidal lymphocytic infiltration [30], production of thyroid auto-antibodies, and subsequently altered function of the B- and T-cell function. However, hypothyroidism is believed to be caused by cellular immunity (Th1), whereas GD follows humoral immunity (Th2).…”
