Immunosuppressants in the Treatment of Pemphigus

Abstract: Summary The results are presented of parenteral treatment with methotrexate (12·5 mg. twice a week) in 19 cases of various forms of pemphigus; small doses of corticosteroids (usually 8–32 mg. daily) were given in addition. Two other cases, both of pemphigus erythematosus, were treated with azathioprine. The response to immunosuppressants was good, chiefly in pemphigus erythematosus. In pemphigus vulgaris. cutaneous lesions responded much better than mucosal ones, which in some cases improved only some weeks af… Show more

“…196 In total, 13 case series have detailed its efficacy. [121][122][123][124][125][126][127][128][129][130][131][132]197 The available data on 116 patients with pemphigus vulgaris demonstrated a clinical improvement in 96 (83%)of the patients. [121][122][123][124][125][126] In patients with severe to moderately severe pemphigus vulgaris, methotrexate may be a useful maintenance therapy once control is achieved with steroid therapy, while allowing a lowering of the corticosteroid dose.…”

“…Jablonska et a!. [10] reported good results with azathio prine alone in two cases of pemphigus erythematosus. Krakowski et al [11] was able to withdraw steroids in a patient with pemphigus, in whom the treatment was continued with azathioprine alone.…”

“…These drugs include alkylating agents such as cyclophosphamide [8,12], folic acid antagonists such as methotrexate [10,13,14,16,17,21], and purine analogues such as azathioprine [2, 3, 6, 7, 10, 11, 18-20, 25, 27], There is little doubt that these immunosuppressive agents usually have a beneficial effect in the treatment of pemphigus and pemphigoid. Most authors recommend the combined administration of a corticosteroid and an immuno suppressive drug, the latter permitting a significant reduction in steroid dosage.…”
