INTRODUCTION Non-communicable diseases such as diabetes and high blood pressure are a heavy health and economic burden in low-income countries. This work aimed to study the prevalence of diabetes, arterial hypertension, and obesity as well as their associated factors in the Niakhene commune in rural Senegal. METHOD This was a cross-sectional, descriptive, and analytical survey among subjects aged at least 18 years and residing in the municipality of Niakhene carried out in October 2020. A systematic random survey, stratified according to sex and sections was carried out. The questionnaire was constructed from the framework of the STEPS 2015 World Health Organization tools and a literature review. This questionnaire was associated with anthropometric, blood sugar and blood pressure measurements. Descriptive and analytical analyzes were performed with R 4.0.2 software. RESULTS A total of 300 individuals were surveyed. They had a mean age of 35.3 years (SD:16.9), female (52.3%), married (65.7%), and uneducated (67.7%). The main family history was high blood pressure (42.0%), diabetes (9.0%), and stroke (5.7%). The prevalence of diabetes was 3.7% including 0.7% newly diagnosed. Factors associated with diabetes were advanced age over 60 (ORaj =10.6 95% CI [1.32-232]) and family history of diabetes (ORaj =10.6 95% CI 8 .77 [1.16-65.7]). On arterial hypertension, the prevalence was 47.7% of which 16.3% were newly diagnosed. It was associated with age and areas of overweight-obesity. Subjects aged 40 to 59 had a 4.1 ORaj (95% CI [1.9-9.3]) times greater risk of being hypertensive and this risk increases with age and people aged 60 and over had a risk of 5.6 ORaj (95% CI [2.37-13.9]). Overweight and obese people with a BMI greater than 25 Kg/m2 were 2.6 ORaj (95% CI [1.25-5.76]) times more at risk of being hypertensive Overweight was noted in 10.7% and obesity in 3.7%. They were mainly associated with age with people aged 25-39 being 7.5 (95% CI [2.35-33.9]) more at risk than young people aged 18-25. CONCLUSION The increasing development of major cardiovascular risk factors should lead health authorities to give crucial importance to the fight against the burden of non-communicable diseases in the context of epidemiological transition.

The prevalence of type 2 diabetes and moderate fasting hyperglycemia is high in Guinea, while data on gestational diabetes are scarce. Objective: To determine the prevalence of Gestational Diabetes among women admitted for prenatal consultation, identify their socio-epidemiological characteristics and any associated risk factors. Patients and Methods: Prospective study conducted in pregnant women who came for prenatal follow-up in two urban health centers in the prefecture of Kindia, Guinea during the period from January 1 to June 30, 2018, screened according to the recommendations of International Association of Diabetes and Pregnancy Study Group (IADPSG). A single abnormal value is sufficient to make the diagnosis of GDM. All pregnant women between 24 and 28 SA were included. Women with multiple pregnancies and/or preexisting diabetes or refusal to participate in the study were excluded. The variables analyzed: maternal age, gestational age, parity and exposure or not to risk factors. The data were analyzed by specific statistical tests with a probability p<0.05. Results: Seven hundred and forty-two (742) pregnant women were screened with 103 cases of GDM diagnosed, representing a prevalence of 13.8%. The epidemiological profile of women with GDM was dominated by the average maternal age which was 28.7 ± 6.5 years, housewives 57.28% (59/103), no level of education 53.39% (55/ 103), married 67.96% (70/103) with an average parity of 2.11± 1.9. The average gestational age was 27.1 ± 5.6 SA. The risk factors were dominated by advanced maternal age, with a proportion of 70.27% and 13.41% of patients without risk factors, screening for GDM was positive (P < 0.000001). Conclusion:The GDM is frequent among the women followed at the prenatal consultation in Kindia. Systematic screening integrated with prenatal care remains the best means of prevention.

The prevalence of gestational diabetes mellitus (GDM) has been steadily increasing over the past years. It is a major risk factor for glucose intolerance and type 2 DM (T2DM). The American Diabetes Association recommends that women whose pregnancy was complicated by GDM be screened for persistent glucose abnormalities at six to 12 weeks postpartum with either a fasting plasma glucose test alone or with a fasting 75-g, two-hour oral glucose tolerance test. This study aimed to identify the main predictive factors of glucose tolerance disorders in early postpartum women with a recent history of GDM. In this retrospective descriptive study, we identified 400 women who met the eligibility criteria for the study. The mean age was 34.54 ± 5.51 years. A total of 70% had a family history of DM, 16% had a personal history of GDM, and 23% had fetal macrosomia in previous pregnancies. The overall incidence of postpartum carbohydrate tolerance disorders was 36.4%, including 12% prediabetes and 24.4% DM. The prevalence of prediabetes and T2DM after delivery was higher with older maternal age, multigravidity, a higher BMI, a history of GDM, and fetal macrosomia in previous pregnancies. Furthermore, the persistence of this impaired glucose tolerance in postpartum was associated with a higher term of diagnosis, a higher glycated hemoglobin (HbA1c) percentage (the discriminant cutoff value with the best sensitivity/specificity ratio was 5.25%), the use of insulin therapy, cesarean section delivery, and fetal macrosomia. After adjusting for confounders, only prior GDM, a higher HbA1c level, macrosomia, and gestational term were found to significantly affect postpartum glucose tolerance. Although postpartum screening for T2DM is recommended for all women with GDM, a significant number of patients fail it. A better knowledge of predictive factors for this outcome is therefore needed for a more effective and targeted medical intervention.
