Impact of a health alert and its implementation on flutamide prescriptions for women: an interrupted time series analysis

Abstract: Background Off-label drug use among ambulatory patients is often based on little or no scientific support. This paper reports the impact of a health warning about the risks of off-label flutamide use by women and the actions subsequently implemented by the public health service targeting such use. Method The study was undertaken in a region in north-west Spain. We designed a segmented regression model of an interrupted time series, in which the dependent variable was the monthly value of defined daily doses o… Show more

“…Since the same outcome can be used to assess both intended and unintended impact, relying only on study findings might also include unintended impact that was anticipated when planning the study. For example, in 2 publications that assessed interventions targeting only females, male subjects were also included in the analysis, and the results were stratified by sex 37,38 . Since the authors did not clarify why male subjects were included in the study, it is unclear whether this subgroup was selected as a control group assuming only secular change or as a group to check possible spill‐over effects.…”

“…The impact of regulatory safety warnings from regulatory bodies from Europe and the United States as well as for Spain has been evaluated in a few systematic reviews and studies that have provided mixed results, many of them showing no or modest effect. Even if their interpretation is hindered by the heterogeneity of the studies in terms of design, setting, outcomes analysed and methodological quality, there is a common recognition that many, multilevel and contextual factors may be mediating between the issue of warnings and their impact in clinical practice, such as for instance the very nature of the alert and the risks involved, the implementation of warning-related interventions by the health services, promotional activity or media coverage ( Carracedo-Martínez and Pía-Morandeira, 2010 ; CarracedoMartínez and Pía-Morandeira, 2011 ; Carracedo-Martínez et al, 2012 ; Dusetzina et al, 2012 ; Piening et al, 2012 ; Carracedo-Martínez and Pia-Morandeira, 2016 ; Goedecke et al, 2018 ; Hurtado-Navarro et al, 2019 ; Georgi et al, 2020 ; Vázquez-Mourelle et al, 2020 ).…”

“…The regular mesh used in this case is simpler and more effective than the irregular mesh and has a stable rendering rate in practice. For example, terrain data is stored as a 2D height map in the framework designed by the algorithm, which can be filtered into a mipmap pyramid containing Llayer after effective processing [11][12].…”
