Interventions to Prevent Unintentional Injuries Among Adolescents: A Systematic Review and Meta-Analysis

Globally, every day, ∼2,300 children and adolescents succumb to unintentional injuries sustained from motor vehicle collisions, drowning, poisoning, falls, burns, and violence. The rate of deaths due to motor vehicle injuries in adolescents is 10.2 per 100,000 adolescents. We systematically reviewed published evidence to identify interventions to prevent unintentional injuries among adolescents aged 11–19 years. We defined unintentional injuries as a subset of injuries for which there was no evidence of predetermined intent, and the definition included motor vehicle injuries, suffocation, drowning, poisoning, burns, falls, and sports and recreation. Thirty-five studies met study eligibility criteria. The included studies focused on interventions to prevent motor vehicle injuries and sports-related injuries. Results suggest that possession of a graduated driver license (GDL) significantly reduced road accidents by 19% (relative risk [RR]: .81; 95% confidence interval [CI]: .75–.88; n = 5). There was no impact of GDL programs on incidence of injuries (RR: .78; 95% CI: .57–1.06; n = 2), helmet use (RR: 1.0; 95% CI: .98–1.02; n = 3), and seat belt use (RR: .99; 95% CI: .97–1.0; n = 3). Sports-related injury prevention interventions led to reductions in the incidence of injuries (RR: .66; 95% CI: .53–.82; n = 15), incidence of injury per hour of exposure (RR: .63; 95% CI: .47–.86; n = 5), and injuries per number of exposures (RR: .79; 95% CI: .70–.88; n = 4). Subgroup analysis according to the type of interventions suggests that training ± education and the use of safety equipment had significant impacts on reducing the incidence of injuries. We did not find any study focusing on interventions to prevent suffocation, drowning, poisoning, burns, and falls in the adolescent age group. The existing evidence is mostly from high-income countries, limiting the generalizability of these findings for low- and middle-income countries. Studies evaluating these interventions need to be replicated in a low- and middle-income country–context to evaluate effectiveness with standardized outcome measures.

Mild traumatic brain injury, commonly known as a concussion, has gained widespread public attention. Approximately 1.1 to 1.9 million children ≤18 years old suffer sport/recreation-related concussions in the U.S. annually. The purpose of this review was to assess research articles examining concussion knowledge of youth sport athletes, coaches and parents. Twenty-one articles published from 2009-2016 were selected. The results of this review suggest that most athletes, coaches, and parents had good knowledge regarding the definition of a concussion, common signs/symptoms, and complications from repeated concussions, while most stakeholders had moderate knowledge about the causes, prevalence, and return-to-play guidelines. However, the majority of respondents did not correctly identify the emotional signs/symptoms associated with concussions, and did not know that youth concussions should be managed more conservatively than adult concussions. Notable knowledge gaps were found among youth athletes and parents compared to coaches. Suggestions to improve these youth sport stakeholders’ concussion knowledge are provided.

Patient characteristics, mechanisms of trauma and imaging results were retrieved, and the occurrence of motor deficits, language impairment, cerebrospinal fluid leak seizures or death was assessed during the whole hospital stays. The most affected age range was that of infants (41.5%), the most frequent reported cause was fall (76.5%) and most children were victims of mild brain injury (96%). A total of 11.7% of the computerized tomography examinations showed traumatic injuries, with fractures in 43.6% of them. Thirteen children presented with the above-mentioned endpoints of morbidity and mortality. Seizure (n=9) was the most frequent one. The age range with the highest incidence was also that of infants (n=6). For the same endpoints, the clinical morbid-mortality was 4.6% for the wholes series and 44,4% for moderate and severe trauma. The mortality rate at the end of the period of hospitalization was 1.1%. According to the data from the present study, falls were the main reported cause of pediatric head trauma. A total of 4.6% of victims presented with neurological endpoints of morbid-mortality during the hospital stay and seizure was the most frequent one. resumo o impacto socioeconômico de crianças portadoras de sequelas de traumatismo crânio-encefálico (TCE) é um importante problema de saúde pública. Neste estudo retrospectivo, foram avaliadas as causas, a morbidade neurológica e a mortalidade do TCE pediátrico, durante o período de permanência hospitalar em um centro de referência no Nordeste do Brasil. Foi examinada uma amostra aleatória de 282 prontuários em um período de doze meses. As características das vítimas, os mecanismos de trauma e os resultados dos exames de imagem foram analisados. A ocorrência dos seguintes desfechos clínicos neurológicos foi contabilizada durante toda a internação, a saber: déficits motores, distúrbios de linguagem, convulsões, fístulas liquóricas clinicamente detectáveis ou morte. A faixa etária mais afetada foi a dos lactentes (41,5%), a causa mais frequentemente relatada foi queda (76,5%) e a maioria das crianças foi vítima de TCE leve (96%).
