Impact of Baseline Steroids on Efficacy of Programmed Cell Death-1 and Programmed Death-Ligand 1 Blockade in Patients With Non–Small-Cell Lung Cancer

Abstract: Baseline corticosteroid use of ≥ 10 mg of prednisone equivalent was associated with poorer outcome in patients with non-small-cell lung cancer who were treated with PD-(L)1 blockade. Prudent use of corticosteroids at the time of initiating PD-(L)1 blockade is recommended.

“…Finally, this study did not include analysis related to steroid use. Baseline steroid use in a similar study setting has been previously reported to be a negative prognostic factor . However, during the observation period of this study, no patient received concurrent systemic corticosteroids (equivalent to greater than 10 mg of prednisone per day) during ICI therapy as a result of our institutionally established local ICI dosing criteria.…”

“…The management of irAEs is based on clinical experience and requires corticosteroid immunosuppression as first‐line therapy . Although the impact of immunosuppression on the overall survival of patients is unclear, the possibility that steroids may adversely impact on antitumour responses and/or overall survival remains a consideration …”
