Impact of chemotherapy-associated liver injury on tumour regression grade and survival in patients with colorectal liver metastases

Abstract: CALI was not directly related to survival. CALI was, however, associated with diminished complete tumour response, and diminished complete tumour response, in turn, was associated with decreased survival.

“…Both colorectal carcinoma and HCC had Grade 3 toxicity rates above the group mean for several lab values. This finding reflects baseline hepatotoxicity from previous chemotherapy used for colorectal carcinoma and underlying cirrhosis in HCC (18)(19)(20). The 1.1% incidence of likely radiation-induced liver disease (7/614 patients) is slightly lower than the 4% rate described by Kennedy et al (13).…”

“…Even if not associated with prognosis, some studies highlighted an inverse relationship between CALI and response to chemotherapy (tumor regression is less evident in patients with severe injuries). 3,13 NEW OUTLOOKS: FUNCTIONAL IMAGING AND TEXTURE ANALYSIS Hepatobiliary scintigraphy (HBS) with 99m Tc-labeled iminodiacetate derivatives could open new perspectives. In staged procedures, HBS demonstrated accurate evaluation of liver function, outperforming volumetric assessments and liver function tests.…”

“…It aims on removing all the tumour tissues from the patient's body, while sparing healthy tissues to preserve function. The intraoperative decisions made by the surgeon to perform the resection are critical for patient survival (9)(10)(11). Unsuccessful surgeries with potential remaining tumour tissues inside the patient can lead to reoperations or heavy complementary treatments such as radiotherapy or chemotherapy affecting the quality of life and the survival of the patient.…”

“…Tissues are then fixed in formalin for hours for optimal histology and long term storage at room temperature. The distance between the tumour and the resection cut is referred as resection margin width and has been reported as an independent prognostic factor on patient's outcome for several malignancies (9)(10)(11)22). The distance between the inked margin and the first encountered tumours cells are then inspected microscopically to define the success of the surgery ( fig.…”
