Impact of graft‐to‐recipient weight ratio on small‐for‐size syndrome following living donor liver transplantation

Abstract: The incidence of SFSS in grafts with a GRWR <0.8 is more than in GRWR ≥0.8; however, the low GRWR does not appear to impact perioperative outcomes or graft survival.

“…If the estimated graft liver volume to standard liver volume (GV/SLV) ratio after the tumor resection is more than 40%, then the remnant liver after resection of the hemangioma can be used for transplantation. GRWR is also an important factor for pediatric LDLT, which should be more than 0.8%[ 20 , 21 ]. In the present case, the GV/SLV ratio was 76.84% and GRWR was 2.1%, which were safe and sufficient indicators for LDLT.…”

“…5 Therefore, small adults may be increasingly disadvantaged for receiving a size-matched whole liver graft. 6,7,13 Additionally, allocation rates of small adults may be negatively influenced due to competition with the pediatric waitlist. However, this potential effect is not quantifiable based on current data.…”

“…Umera et al have also demonstrated that after graft inflow modulation the GRBWR can be successfully reduced to below 0.7 . In a recent meta‐analysis analyzing, the impact of low GRBWR on the incidence of small‐for‐size syndrome (SFSS), Bell et al showed that although the incidence of SFSS, was higher (10% vs 5%, P = .02) in the GRBWR < 0.8 group compared to GRBWR ≥ 0.8 group, there was no difference in either early post‐transplant mortality or long term graft and patient survival …”
