Objectives:To analyze the adequacy of hemodialysis treatment in patients with arterial stiffness and to associate the effects of renal disease with sociodemographic and clinical factors. Method: A cross-sectional epidemiological study with 149 chronic renal patients undergoing hemodialysis treatment, in the interior of the State of São Paulo. They used to measure arterial stiffness with the Dyna-MAPA  device. The tests statistical were multivariate linear regression, t Student and chi-square were applied to the biomarkers and the occurrence of arterial stiffness. Results: Most were of productive age, professionally inactive, male, in a stable union, water, and food restriction, and with less than 48 months of dialysis treatment and 36.9% of patients had arterial stiffness. The biomarkers creatinine, urea, and phosphorus were increased, hematocrit and hemoglobin were decreased, mean interdialytic weight was 2.34 ± 1.4kg, and most of the Kt / V inadequate. The effects of renal disease were statistically significant with those enrolled in the transplant list, longer treatment time, and increased creatinine. Conclusion: These results are expected to reduce cardiovascular morbidity and mortality in these patients. RESUMOObjetivos: Analisar a adequação do tratamento hemodialítico em pacientes com rigidez arterial e associar os efeitos da doença renal com os fatores sociodemográficos e clínicos. Método: Estudo epidemiológico, transversal realizado com 149 renais crônicos em tratamento de hemodiálise, no interior do Estado de São Paulo. Utilizou para medida da rigidez arterial o aparelho de Dyna-MAPA  . Foram aplicados os testes de regressão linear multivariada, t Student e o qui-quadrado para os biomarcadores e a ocorrência de rigidez arterial. Resultados: A maioria estava em idade produtiva, inativa profissionalmente, do sexo masculino, em união estável, fazendo restrição hídrica e alimentar e com menos de 48 meses de tratamento dialítico e 36,9% dos pacientes apresentavam rigidez arterial. Os biomarcadores creatinina, ureia e fósforo estavam aumentados, hematócrito e hemoglobina estavam diminuídos, média de peso interdialítico de 2,34 ± 1,4 kg, e a maioria do Kt/V inadequado. Quanto aos efeitos da doença renal observou-se significância estatística com os inscritos na lista de transplantes, maior tempo de tratamento e aumento da creatinina. Conclusão: Espera-se que esses resultados proporcionem a diminuição da morbimortalidade cardiovascular desses pacientes. BackgroundHand grip strength (HGS) is a key measurement in the assessment of frailty phenotype in haemodialysis patients. However, the measurement is not very standardized, and notably, current data on the potential impact of a haemodialysis session on the results are both limited and controversial. In the present analysis, we compared HGS results before and after a haemodialysis session in 101 patients.MethodsIn the current observational study, HGS has been measured in adult haemodialysis patients on the same day, first before connection to the dialysis machine and then just after disconnection. At each timing, measurements were repeated three times with an interval of 5 s between measurements and the higher value was used for analysis.ResultsOne hundred and one patients (64% men) with a median (interquartile range, 25th percentile; 75th percentile) age of 66 (46; 76) years were included. In the whole population, a significant decline in HGS was observed after dialysis, with an absolute median decline of − 4 (0; −6) kg and a relative median difference of −11 (0; −20)%. These differences were observed in both genders and were independent of the baseline HGS value.ConclusionsOur results suggest that the timing (before or after the dialysis session) of hand grip assessment is clinically relevant and should be taken into account in clinical practice and also in epidemiological and clinical studies.

ABSTRAKPendahuluan. Salah satu faktor utama yang terlibat dalam gangguan mineral tulang dan muskuloskeletal pada pasien penyakit ginjal kronik (PGK) adalah fibroblast growth factor-23 . Peningkatan kadar FGF-23 terjadi pada awal PGK dan semakin meningkat pada PGK tahap akhir terutama yang menjalani dialisis. Fibroblast growth factor-23 perlu mendapat perhatian khusus karena perannya terhadap otot skeletal pada pasien PGK yang menjalani hemodialisis belum diketahui secara pasti. Penelitian ini dilakukan untuk mengetahui korelasi antara kekuatan genggam tangan (KGT) dengan kadar FGF-23 pada pasien yang menjalani hemodialisis dua kali seminggu.Metode. Penelitian ini merupakan studi potong lintang yang dilaksanakan di Unit Hemodialisis Rumah Sakit Dr. Cipto Mangunkusumo Jakarta, pada pasien yang mendapatkan dialisis 2 kali seminggu. Pengukuran FGF dengan pemeriksaan intact fibroblast growth factor-23 (iFGF-23) dan menilai KGT dengan dinamometer hidraulik tangan merek Jamar. Pemilihan subjek dilakukan secara consecutive sampling sampai jumlah subjek yang diperlukan terpenuhi. Hasil.Subjek pada penelitian ini sebanyak 74 subjek dengan kadar pemeriksaan FGF-23 pada laki-laki (median 3.276 pg/ml [rentang 41-6.613 pg/ml]) lebih tinggi dibandingkan dengan perempuan (median 1.585 pg/ml [rentang 21-4.820 pg/ml]). Nilai median KGT pasien laki-laki adalah 25 kg (rentang 11-48 kg), sedangkan pada perempuan adalah 20 kg (rentang 8-26 kg). Setelah dilakukan penyesuaian dengan indeks komorbid charlson modifikasi dan indeks massa tubuh, tidak didapatkan korelasi antara FGF-23 dengan KGT pada subjek laki-laki (r=-0,053, p=0,7), akan tetapi terdapat korelasi negatif bermakna pada subjek perempuan (r=-0,4, p=0,02).Simpulan. Kadar FGF-23 memiliki korelasi negatif bermakna dengan KGT pada perempuan dengan IMT >18,5 kg/m 2 dan hal tersebut tidak ditemukan pada subjek penelitian laki-laki.Kata Kunci: FGF-23, hemodialisis, kekuatan genggam tangan ABSTRACT Introduction.
