Impact of CYP2D6 Genotype on Amitriptyline Efficacy for the Treatment of Diabetic Peripheral Neuropathy: A Pilot Study

Abstract: Hence, CYP2D6 genotype contributes to treatment outcome and may be useful for guiding drug therapy. Future investigations in a larger patient population are planned to support these preliminary findings.

“…Again detecting these variants relies on large discovery cohorts, with sufficient statistical power. Because of this difficulty, very few pharmacogenetics studies have been conducted in NeuP to date (Brasch-Andersen et al., 2011, Chaudhry et al., 2017). Sensory phenotype stratification, together with classifying patients depending on genetic risk, has the potential to lead to personalized medicine to determine the best course for treatment.…”

“…It is known to inhibit the presynaptic reuptake of serotonin (5-HT) and norepinephrine (NE) and thus increase the concentrations of both neurotransmitters at the synaptic cleft (12) . Some of the side effects for amitriptyline include anticholinergic effects such as constipation, dizziness, dry mouth, blurred vision and urinary retention, besides weight gain, sexual dysfunction, orthostatic hypotension and cardiotoxicity (13) (14) .…”

“…Whereas CYP2C19 influences the ratio of metabolites, CYP2D6 plays a greater role in drug clearance. This may lead to raised plasma concentrations of amitriptyline and nortriptyline in CYP2D6 PMs or in patients co-administered a CYP2D6 inhibitor [ 46 ]. CPIC guidelines therefore recommend a lower starting dose when used at dosing ranges for depression [ 47 ].…”
