Impact of interval from breast conserving surgery to radiotherapy on local recurrence in older women with breast cancer: retrospective cohort analysis

Abstract: Objectives To determine if the length of interval between breast conserving surgery and start of radiotherapy affects local recurrence and to identify factors that might be associated with delay in older women with breast cancer. Design Retrospective cohort analysis with Cox proportional hazards models to study the association between time to radiotherapy and local recurrence, and propensity score and instrumental variable analyses to confirm findings. Logistic regression investigated factors associated with l… Show more

“…39 A delay in delivery of radiotherapy either because of long waiting lists or because chemotherapy is given fi rst, could jeopardise its eff ectiveness. 40 A recent large study 41 in 18 050 patients has shown that timeliness does make an important diff erence. If radiotherapy is delivered intraoperatively, it would avoid a temporal miss 42 and enable the shortest possible interval between surgical resection of the cancer and accurate delivery of radiotherapy.…”

“…[20][21][22][23][24] It also allows normal breast landmarks to be preserved, 30 Delays in the administration of radiotherapy have been reported in patients undergoing immediate reconstruction. 31 Research from patients undergoing breast conservation has illustrated a correlation between increasing intervals between surgery and radiation and the risk for local recurrence, 32 and other studies have shown increased mortality risk if the interval between chemotherapy and radiation therapy exceeds 6 months. 6 Furthermore, the presence of an autologous flap or an implant may reduce the capacity to deliver effective chest wall radiation.…”
