Impact of minimally invasive extracorporeal circuits on octogenarians undergoing coronary artery bypass grafting. Have we been looking in the wrong direction?†

Abstract: The MiECC concept has shown its benefits regarding both morbidity and mortality in this high-risk patient population. We believe that this beneficial effect finds its reason in a better preservation of physiologic reserves that are essential for a positive outcome in this patient group.

“…Previous studies of patients undergoing coronary surgery or simple valve surgery with the use of a MiECC revealed a relatively short mean ECC time (ranging from 74.9 to 112 minutes) and a mean aortic clamp time (ranging from 49.9 to 86 minutes) 5,6,15‐18 . Our overall mean ECC time (159 minutes) and overall mean aortic clamp time (109 minutes) were comparatively long but akin to the reported mean ECC times (144‐181 minutes) and aortic clamp times (90‐107 minutes) recorded during ATAAD surgeries performed under mild‐to‐moderate hypothermia 21‐23 .…”

“…Therefore, there remain no clear guidelines regarding the optimal transfusion volume. Whether MiECC can reduce the transfusion volume in ATAAD surgery, as it does in coronary surgery, 5‐8 remains a question. Chu et al assessed the incidence of blood transfusion in patients undergoing proximal thoracic aortic surgery (85 elective cases and 53 emergency cases) 29 .…”
