Impact of pretreatment second look 18FDG-PET/CT on stage and treatment changes in head and neck cancer

Abstract: Highlights The potential value of a second look PET/CT prior to radiotherapy was investigated. New lymph node metastases were discovered in half of the patients. Progression of the primary tumor and distant metastases were also observed. This strategy led to changes in treatment planning in more than half of the cases.

“…Tissue changes from previous local therapies strongly impede target volume delineation in recurrent head and neck cancer. Combining multi-modality imaging including CT, diffusionweighted MRI and positron emission tomography (PET)/ CT can be crucial to mitigate uncertainties in contouring [32,33]. One patient with type E local failure developed tumor recurrence within 5 mm of the CTV.…”

“…The radiological lymph node metastasis (RLNM) was determined in MRI by two maxillofacial surgeons (L.Z.N. and Y.L., with more than 10 years of experience) blinded to the patients' clinical outcomes, according to the radiological diagnostic criteria reported by Yoon et al 15 The radiological criteria had been reported for the diagnosis of CLNM in patients with head and neck squamous cell carcinoma (HNSCC) 16,17 . The detailed diagnostic criteria for RLNM were presented in Data S1, Supporting Information.…”
