Impact of Socioeconomic Adjustment on Physicians’ Relative Cost of Care

Abstract: Background Ongoing efforts to profile physicians on their relative cost of care have been criticized because they do not account for differences in patients’ socioeconomic status (SES). The importance of SES adjustment has not been explored in cost profiling applications that measure the costs using an episode of care framework. Objectives We assessed the relationship between SES and episode costs and the impact of adjusting for SES on physicians’ relative cost rankings. Research design We analyzed claims … Show more

“…The number and year‐over‐year change in RRT versus Code Blue activations for each individual inpatient unit starting November (quarter [Q] 3) 2007 through 20011 were compared using linear regression and described by Pearson correlation coefficient . Patient acuity over the course of observation was monitored hospital wide through case mix index (CMI) . StatsDirect (StatsDirect Software Inc., Ashwell, UK) statistical software was used for all comparisons.…”

“…Overall patient acuity over the course of the study was monitored hospital wide through case‐mix index (CMI). The index is based upon the allocation of hospital resources used to treat a diagnosis‐related group of patients and has previously been used as a surrogate for patient acuity . The code RN who performed the resuscitation is responsible for entering data into a protected performance improvement database.…”

“…This index has been used in previous studies to estimate SES and has been validated using factor analysis. 19 , 20 To improve precision of the estimates for each ZCTA, we used 5-year averages from 2009 to 2013. We then merged these SES variables at the patient level using each patient’s ZCTA of residence which is available in the HCUP databases.…”
