Impacts of cytogenetic categories in the Revised International Prognostic Scoring System on the prognosis of primary myelodysplastic syndromes: results of a single-center study

Abstract: Recently, the Revised International Prognostic Scoring System (IPSS-R) has been developed for assessing the prognosis of primary myelodysplastic syndromes (MDS) and has shown satisfactory outcomes for prognostic stratification. In this new system, cytogenetics remains the key stratification parameter and karyotypic abnormalities are classified into five prognostic subgroups with more uncommon cytogenetic subsets. Using this system, we analyzed the cytogenetic features of 532 adult Chinese patients with primary… Show more

“…Recently, a revised version of the IPSS score taking into account patient's transfusion dependency has been proposed [25]. Transfusion dependency is defined as administration of more than two red pack-cells a month for two consecutive months.…”

“… 11 However, it accounts for about 30–35% in Chinese patients with abnormal karyotype and represents 14–20% of MDS patients in Western countries. 12 13 14 Although tr8 as sole anomaly is categorized as intermediate IPSS cytogenetic subgroup, the outcome varies greatly both in terms of OS and risk of evolution to AML. 5 15 16 Furthermore, patients with trisomy 8 are more likely to improve hematologically with immunosuppressive treatment (IST) compared with patients with other forms of MDS.…”

“…There are few studies that have systematically analyzed the clinical prognostic factors of primary MDS with tr8 in Chinese patients. 12 13 15 In this study, we aimed to assess clinical features and identify prognostic factors in Chinese primary MDS with tr8 anomaly, and compare the prognostic value of IPSS and IPSS-R in this group by our single institution cohort.…”

“…As we can notice, these studies showed controversies about the frequency of abnormal karyotypes in hypocellular MDS. Several studies have demonstrated the prognosis value of cytogenetic analysis in MDS [10, 11, 13, 14]. So, the karyotype was incorporated in prognostic scores, allowing risk group stratification and helping to choose the therapy like the International Scoring System for Evaluating Prognosis (IPSS).…”
