“…Second, discussions about transition should take place between CAMHS and AMHS at least 6 months before it occurs, accompanied by: (1) clear protocols and pathways related to transition, (2) a well‐structured, patient‐centred care plan, (3) formal handover of care in at least one specific multidisciplinary discharge planning meeting, (4) an overlap period of joint working by both services during the transitional phase, and (5) good working relationships between services and an understanding of each other's way of working.
