Every year, the frequency of use of biological prostheses for treatment of valvular heart disease increases.The implantation of a biological prosthesis allows avoiding lifelong anticoagulant therapy, but at the same time it may require further replacement due to the dysfunction development.Traditional reoperation is associated with a certain risk of trauma to various structures and cardiac chambers during explantation of non-functioning valve.Transcatheter replacement of the prosthesis (“valve-in-valve”) is not always realizable due to lack of routes for the delivery system and small diameter of the initial valve.“Valve-in-valve” is not always demanded because it does not provide an option to choose mechanical or biological prosthesis.Valve-on-valve implantation technique implies that the new prosthesis is implanted in the carcass of the previous valve. It allows avoiding severe intraoperative complications, reducing the duration of the main stage of the surgery and adverse events in the early postoperative period, and provide an option to choose implantable device.In this review, the authors attempted to comprehensively evaluate the world experience in applying the “valve-on-valve” technique.

A 73-year-old woman admitted to our hospital with shortness of breath and edema of the lower extremities was diagnosed with right ventricular failure stemming from tricuspid valve regurgitation. She had undergone mitral valve replacement (MVR) with a mechanical valve at the age of 51, and reoperative MVR with mechanical valve, tricuspid valve replacement (TVR) with bioprosthetic valve, and pacemaker implantation at the age of 63. Reoperative TVR was performed when the patient failed to respond to drug therapy. A beating heart cardiopulmonary bypass procedure was performed in which only the bioprosthetic valve leaflet was excised, and reoperative TVR was performed with a 27-mm OptiForm mechanical mitral valve (Sulzer Carbomedics Inc., Austin, TX, USA) by the valve-on-valve technique. The operative course was uneventful. The technique used here appears to be an effective approach to reoperative TVR, in this instance making it possible to avoid the risks associated with excision of the old prosthesis.

A coconut left atrium 23 years after mitral valve replacement for chronic mitral stenosis

We present the case of a 65 year-old female who was admitted to the hospital because of deterioration of chronic dyspnea. Twenty-three years prior to this admission, mitral valve replacement for chronic mitral stenosis was performed using a Starr-Edwards caged-ball prosthesis. There was severe pulmonary hypertension. On transthoracic echocardiography and on cineradiography, the function of the mitral valve prosthesis was unimpaired. However, cineradiography showed extensive mural calcification of the left atrium; we report this remarkable finding.
