Implantation of a novel tissue-engineered graft in a large tendon defect initiated inflammation, accelerated fibroplasia and improved remodeling of the new Achilles tendon: a comprehensive detailed study with new insights

Abstract: We constructed a new artificial collagen-based graft as a tendon proper and covered it with a polydioxanone sheath. This bioimplant was tested in vitro and also its effectiveness was tested in a large tendon defect model in vivo. A 2-cm full defect in the left Achilles tendon of all animals (n = 120) was created. The animals were andomly divided into three groups: control (n = 40), treated with collagen-based graft (n = 40) and treated with collagen-Polydioxanone-based graft (n = 40). Clinical examination was … Show more

“…3 Without an appropriately robust inflammatory response, effective healing cannot occur. 30 During the inflammation, the inflammatory cells infiltrate the injured area, and regulate several events. Neutrophils and macrophages exert a powerful phagocytic activity and lyse the necrotic tissues and microorganisms.…”

“…Transverse sections with 70-80 nm thickness were prepared, and standard methods were employed for production of the ultramicrographs (number of tendon samples in each group ¼ 10 left, 10 right, number of ultrathin sections in each tendon ¼ 5, number of ultramicrographs in each ultrathin section ¼ 5; in total, 250 L and 250 R ultramicrographs in each group). 30 The number of collagen fibrils and elastic fibers were counted in all the ultramicrographs (magnification 39,000Â). The transverse diameter of all the collagen fibrils of all the ultramicrographs and the transverse diameter of one hundred elastic fibers were measured.…”

“…If small, tendon defects could be sutured directly or reconstructed with autogenous tissues [1,2]. However, reconstruction of large tendon defects with autografts has considerable limitations including graft availability and donor site morbidity and this technique requires double surgery [5][6][7]. Using allografts increases the risk of disease transmission and rejection.…”
