Implementation and evolution of a regional chronic disease self-management program

Abstract: OBJECTIVE: To establish a comprehensive, community-based program to improve and sustain self-management support for individuals with chronic diseases and complement office-based strategies to support behaviour change.PARTICIPANTS: Health service delivery organizations. The group problem solving strategy of goal setting and the adaptation of the Stanford Chronic Disease Self-Management Program (Iyngkaran et al, 2016 ; Liddy et al, 2016 ) was rated well for priority (1.80), but poorly on technical complexity (3.10) and capital intensity (3.10).
