Implementation of laparoscopic hysterectomy for endometrial cancer over the past decade

Abstract: BackgroundLaparoscopic hysterectomy (LH) for the treatment of early-stage endometrial carcinoma/cancer (EC) has demonstrated to be safe in several randomized controlled trials. Yet, data on implementation of LH in clinical practice are limited. In the present study, implementation of LH for EC was evaluated in a large oncology network in the Netherlands.ResultsRetrospectively, a total of 556 EC patients with FIGO stage I-II were registered in the selected years. The proportion of LH gradually increased from 11… Show more

“…The transition from open to minimally invasive surgery has been documented in various countries (16,17). Although our analysis confirms the published data, two events occurred during our observation that should have changed the clinical management, namely the approval of ulipristal acetate in 2012 and the issuing of the FDA warning in 2014.…”

“…Among the patients with stages I and II endometrial cancer treated with surgery ( n = 8548), 28.3% underwent laparoscopic surgery. In the Netherlands, the proportion of patients with stages I and II endometrial cancer undergoing laparoscopic surgery gradually increased from 11% in 2006 to 85% in 2015 5 . Thus, laparoscopic surgery for endometrial cancer is expected to become more widespread in Japan.…”

“…In contrast to our expectations, we found that patients scored higher on physical and role functioning than the normative population at baseline and at 12 months, with scores being comparable at 24 months. Perhaps, our different baseline findings may be partly explained by differences in the proportion of patients receiving laparoscopic hysterectomy because the implementation of laparoscopic hysterectomy increased steeply in the last decade, from 11% in 2006 to 85% in 2015 [38]. The PORTEC-2 trial was conducted between 2002 and 2006 and therefore, compared to our cohort, these patients likely received laparotomy more frequently, which is known to have more impact on functioning than laparoscopic hysterectomy.…”
