Implementation of Recommendations for Hand Eczema Through a Multifaceted Strategy. A Process Evaluation Among Health Care Workers

Abstract: Process data give important insights into how an intervention is implemented. The aim of the present study is to conduct a process evaluation, alongside a randomised controlled trail, on the implementation of recommendations for the prevention of hand eczema. The intervention was carried out in healthcare workers' departments and consisted of working groups and role models. The role models were selected based on their representativeness, their influence on colleagues, and their motivation. The focus of the wor… Show more

“…The percentage of approached stakeholder representatives that attended a working group meeting was 79 %. This reach at organisational level is similar to that reported in other studies using a comparable working group component (Driessen et al 2010 ; van der Meer et al 2014 ; Pehkonen et al 2009 ). It is remarkable that within all participating organisations no management representative was present at the working group meetings.…”

“…This paper describes the process evaluation of the multifaceted implementation strategy of the PA by using the Linnan and Steckler’s framework ( 2002 ). This framework has been applied in comparable studies (Arends et al 2014 ; Driessen et al 2010 ; van der Meer et al 2014 ), and has proved to be a useful guide to conduct process evaluations of implementation studies (Murta et al 2007 ). The aim of this process evaluation was threefold: (1) to examine whether the multifaceted implementation strategy of the PA was carried out as intended, (2) to identify barriers to the application of the PA by supervisors, and (3) to examine the extent to which the PA was applied by supervisors.…”

“…Our study showed that 61% of respondents only rarely apply protective creams (1-2 times per day) despite the fact that their condition requires it. This can be explained by inadequate educational measures ( 18 , 19 ). It is necessary to be aware of the need for protection of the skin barrier with protective creams, especially in case of irritant contact dermatitis wherein future occurrence of lesions or exposure to irritants can be prevented ( 20 ).…”

“…A joint effort and equal input and voice of all working group members in identifying barriers and selecting solutions are important aspects of the PA. With employees (with chronic conditions) or other stakeholders not being able to provide input in every step of the PA, the power of the approach could have been compromised 19 . When comparing our findings to the literature, one study using the PA showed much less deviation from the protocol and adequate dose delivered and dose received 30 . Although another PA study also described less implementation of solutions than initially expected 31 …”

“…Furthermore, the PA enabled OPs to proactively initiate and pursue prevention within an organization, whereas OPs currently largely focus on reducing sickness absence 32 . In contrast, in another PA study, using occupational nurses as process leaders, no doubts on the suitability of the process leader were expressed 30 …”
