Implementation of respiratory‐gated VMAT on a Versa HD linear accelerator

Abstract: The accurate delivery of respiratory‐gated volumetric modulated arc therapy (VMAT) treatment plans presents a challenge since the gantry rotation and collimator leaves must be repeatedly stopped and set into motion during each breathing cycle. In this study, we present the commissioning process for an Anzai gating system (AZ‐733VI) on an Elekta Versa HD linear accelerator and make recommendations for successful clinical implementation. The commissioning tests include central axis dose consistency, profile cons… Show more

“…However, these systems are not as widely available as C‐arm linacs where motion mitigation strategies such as gating are applied in clinical practice 15 . Free‐breathing gating, however, imposes frequent beam‐on/off switching, making it potentially more difficult to deliver for dynamic techniques 18,19 . To apply free‐breathing gating for DTRT, several challenges and questions need to be answered: Is the machine capable of applying free‐breathing gating DTRT for realistic motion traces?…”

“…Others in the literature have reported considerably smaller beam-on latencies than those reported in this work using the modular or end-to-end approaches. 5,8 Within the control system of the LINAC, several parameters can be adjusted to alter the temporal characteristics of gated delivery, particularly with regards to beam-on latency. 9,11 It is expected that the beam-on latencies reported in this work could be reduced by altering these parameters.…”

“…Several authors have presented methods for measuring gating latencies using the displacement of a radio‐opaque moving target captured using film or the LINACs electronic portal imaging device (EPID). 4 , 5 , 6 , 7 Such methods can be used to measure the end‐to‐end latencies of the gating system but do not allow for analysis of the individual components included in the system. Modern gating systems are typically composed of multiple components, or subsystems, each having their own latency.…”

“…The linear accelerator must be characterized before the application of this complex technique to reduce the beam-on delay, to study the efficiency of the delivery, and to investigate the influence of frequent beam interruptions on the dosimetric accuracy of gated deliveries (17)(18)(19)(20)(21) . This technique is time-consuming, and its benefits must be further validated before its application in a clinical scenario.…”
