Implementation science protocol for a participatory, theory-informed implementation research programme in the context of health system strengthening in sub-Saharan Africa (ASSET-ImplementER)

Abstract: ObjectivesASSET (Health System Strengthening in sub-Saharan Africa) is a health system strengthening (HSS) programme involving eight work-packages (ie, a research study that addresses a specific need for HSS) that aims to develop solutions that support high-quality care. Here we present the protocol for the implementation science (IS) theme within ASSET (ASSET-ImplmentER) that aims to understand what HSS interventions work, for whom and how, and how IS methodologies can be adapted to improve the HSS interventi… Show more

“…Thus, the analysis of the data and the results section of this paper are organized according to these two types of factors and with regard to the main themes that emerged during the discussions with a more inductive approach. The analysis of constraining and enabling factors is often used in the study of policies in Africa (Sakyi, 2008 ; Seward et al, 2021 ).…”

“…As an example, these frameworks can be used to theorise how the identified barriers and enablers bring about change by addressing interactions across multiple domains at microlevel, mesolevel and macrolevel. 27 Applying determinant frameworks to data-collection tools (eg, focus group discussions, interviews) in the preimplementation phase of research provides an evidence-based methodology to identify and address contextual and behavioural barriers and enablers that might otherwise have been missed. Identifying determinants in this phase of research can inform the selection of implementation strategies to support delivery of the evidence-informed practice.…”

“…Implementation science determinant and evaluation frameworks were used throughout the study. At the end of the preimplementation phase of the study, we used determinant frameworks to identify the barriers and/or enablers that we may have missed that would further inform the selection of any additional HSS interventions [17]. Speci cally, the Theoretical Domains Framework (TDF) [18] and the Context and Implementation of Complex Interventions Framework (CICI) [19] were used to identify determinants that could potentially in uence implementation efforts throughout the implementation process.…”

“…Findings from the pre-implementation phase of the study were reviewed at an annual ASSET meeting [17]. The TDF [18] and the CICI framework [19] were used to identify contextual and behavioural determinants that could in uence the implementation of HSS interventions and implementation outcomes that may have been missed in the ToC workshop.…”
