Importance of Nutrition to Corneal Grafts When Used as a Carrier of the Boston Keratoprosthesis

Abstract: The Boston KPro design with a back plate containing holes protects the overlying corneal tissue from necrosis and melts. This improved situation is likely due to increased aqueous access and better nutrition to the corneal graft cells. In addition, this study confirms earlier work regarding the particular corneal fragility of patients with autoimmune diseases.

“…9,10 Because of improvements in design and postsurgical management, the use of keratoprosthesis devices and, in particular, the Boston type 1 keratoprosthesis, has increased substantially in recent years. [11][12][13][14][15][16][17] The multicentre Boston type 1 keratoprosthesis study and several recent series have documented success of the Boston type 1 keratoprosthesis, in regard to both retention rate and visual outcomes. [18][19][20][21][22][23][24][25][26] Traditionally advocated for use in patients with profound vision loss in both eyes, recent successful outcomes in patients with preserved vision in the fellow eye have argued for the expansion of its indication.…”

“…Advances in type I BKPro design have improved its retention rate. The latest version approved by the Food and Drug Adminis tration (FDA) for marketing presents 8 holes of 1.3 mm diameter in the back plate to allow aqueous nutrition for the corneal button sandwiched between the front and back plate, protecting the tissue from necrosis and melt (2,9) . Moreover, the addition of a titanium locking ring, on the portion of the front plate protruding from the back plate, helps to prevent latter intraocular unscrewing of the BKPro complex.…”

“…9,10 Second, in 2004, a titanium locking c-ring was added to prevent intraocular disassembly of the device. Third, in 2007, the design was changed from a threaded (screw-type) assembly to a threadless design which simplified assembly and produced less damage to the donor endothelium.…”

“…Life-long topical steroids is necessary in all KPro eyes to prevent inflammation. 10,14 Also, the addition of 5% povidone iodine washes were added in each checkup with patients having high risk of endophthalmitis as well as those with a single eye, self-immune diseases or previous endophthalmitis history.…”
