Importance of transesophageal echocardiography in the critically ill and injured patient

Abstract: Echocardiography, particularly transesophageal echocardiography (TEE), is a vital diagnostic and monitoring imaging modality for the intensivist. The field of echocardiography spans different venues and pathologies, ranging from surface transthoracic echocardiography and portable hand-held echocardiography, to contrast echocardiography, stress echocardiography, and TEE, among others. Numerous investigations have proven the worth of echocardiography, especially TEE, in the critically ill and injured patient, ch… Show more

“…Bedside echocardiography is considered a promising tool for the management of critically ill patients [13], and has been demonstrated to be useful in establishing the diagnosis and provides hemodynamic monitoring [14-16]. However, conventional TTE and TEE are not always readily available in an ICU setting.…”

“…Dyspnea, chest pain, and syncope are key symptoms that can lead to diagnosis, but these symptoms are absent in ICU patients who are under sedation and on mechanical ventilation [4]. In this scenario, after the development of sudden, severe hypotension, PE must be considered and included in a differential diagnosis according to the cause of admission [5]. TEE played a pivotal role in our patient due to the poor echocardiographic window, which is common in ICU patients.…”

“…Furthermore, TEE has been shown to be of additional value in many instances for critically ill patients because of its ability to provide excellent visualization of cardiac structures. In this context, TTE has a diagnostic success rate of 50% and can occasionally lead to inadequate images that are not able to establish a diagnosis, compared with a 90% success rate for TEE [5]. TEE has 70% sensitivity and 81% specificity for the confirmation of PE [8].…”
