Improved growth of preterm infants receiving mother’s own raw milk compared with pasteurized donor milk

Abstract: Mother's own raw milk improves weight gain compared with donor milk in preterm infants. Lactation strategies should be sought that helps mothers to increase their milk production.

“…Interestingly, we observed significantly lower rates of severe complications, sepsis, persistent arterial duct, and BPD in neonates receiving early raw MOM. Similar observations have emerged from other recent studies [17,18,22,23,24]. However, a cause-effect relationship cannot be deduced from this observational study.…”

“…While pasteurisation prevents CMV transmission, it may alter bioactive and nutritional components [14,15]. Moreover, this process might delay the use of MOM and increase the consumption of donor milk or formula [16,17]. Thus, despite the national recommendations, practices regarding the use of raw MOM vary in French NICUs [17,18].…”

“…In this context, the methods of increasing breastmilk production of mothers, nursing and breastfeeding should be investigated for preterm babies in neonatal units. The politics and applications of hospitals in relation with milk production, milking and keeping milk should be examined (25).…”

“…Early introduction of enteral feeding is beneficial for intestinal maturation and protection against NEC, but the quality of the diet may critically affect these positive effects. For example, the feeding of mother's milk matures the preterm gut and reduces the risk of NEC relative to bovine milk-based infant formula (IF) and possibly also banked donor milk (22,29,34,39,40). Donor milk (DM) is normally obtained at a relatively late stage of lactation, and therefore may not be appropriate as the first diet, when infants are supposed to receive mother's colostrum.…”
