Improved Perceptions of Education and Wellness Among General Surgery Residents and Faculty After the Implementation of Outpatient Scribes

Abstract: Background Recent studies have revealed a high rate of burnout among general surgery (GS) residents. Efforts to design and implement interventions to mitigate fatigue in surgical trainees are critical. Our aim was to assess the educational and wellness impact of outpatient scribe utilization at an academic GS residency program. Methods Electronic surveys were sent to 38 GS residents and 15 faculties who used outpatient scribes for at least 12 months. Questions were scored on a 5-point Likert scale with answers… Show more

“…Assistance with clerical burden by hiring patient coordinators, dictation services, and medical scribes reduces resident burnout 70 . After working with outpatient scribes for 12 months, 80% to 90% of general surgery residents felt scribes enabled them to focus more on patient care, reduced workload, and improved well‐being and adherence to duty hour restrictions 71 . Residents and faculty believed outpatient scribes enhanced quality of surgical education by allowing more time for teaching 71 …”

“…71 Residents and faculty believed outpatient scribes enhanced quality of surgical education by allowing more time for teaching. 71 Protected nonclinical time can also improve resident wellbeing. Two hours per week of protected nonclinical time was associated with decreased EE and increased well-being without affecting surgical case or clinic coverage; there were improvements in job stress, control over workload, and time for documentation.…”

“…In recent years, much attention has focused on many aspects of the training process, such as surgical and academic aspects, and residents' various problems [ 9 , 10 , 11 , 12 ]. However, while it has been demonstrated that urology resident involvement is not associated with increased overall and surgical complications [ 13 ], very few studies have evaluated the training of surgical and urology residents from the point of view of the patient, who is a passive participant in their medical education.…”

“…Physicians reported increased satisfaction with medical scribing services, greater overall productivity (patients seen per hour), and cost savings due to reduced time spent charting per patient [14]. Tohmasi et al in an electronic survey study on general surgery residents and faculty using outpatient scribes concluded that the implementation of medical scribes enhanced the residents' education and wellness by mitigating resident fatigue, improving adherence to duty hour restrictions, and allowing faculty and residents more time to focus on patient care and education [16]. Similarly, Gottlieb et al reported that the presence of a medical scribe improved both provider and patient satisfaction [15].…”
