Improvement of Hypothyroidism after Laparoscopic Roux-en-Y Gastric Bypass for Morbid Obesity

Abstract: Improvement of hypothyroidism may be an additional benefit of bariatric surgery that has not been previously reported. Reduction of thyroxine requirements is most likely the result of the decrease in the BMI.

“…Thyroid hormones regulate energy metabolism and affect fat distribution, body weight, and metabolic disorders (4,5). Hypothyroidism is associated with a risk of developing metabolic syndrome, and several groups have shown the relationship between hypothyroidism and NAFLD and hypothyroidism and NASH (6)(7)(8)(9)(10).…”

“…Raftopoulos' series [5] reported that post-LRYGB hypothyroidism improved and thyroxine requirements were reduced in 10 of the 23 patients (43.5%). Two of the 23 patients (8.7%) had complete resolution and the remaining 8 patients (34.8%) had reduction (14%-50%, median 33%) of their thyroxine requirements; 13 of the 23 patients (56.5%) had no change in their thyroxine requirements.…”

“…Improved thyroid status has been reported among obese patients following other bariatric surgical procedures [4][5][6][7][8][9][10][11][12], while few studies have been published on the effect of LSG on thyroid status [13][14][15].…”

“…Gastric bypass seems to restore thyroid function in hypothyroid patients independently of final BMI [20][21][22][23], while gastric banding seems to reduce TSH levels, with no effect on fT3 and fT4 levels [23,24]. This phenomenon is often attributed to the loss in body weight, which seems to correlate to TSH levels; however, changes in the nutritional intake interfering with peripheral fT3 de-iodination and modifications of leptin concentrations seem to play a role in the decrease of TSH secretion.…”
