Improving Hands‐free Speech Rehabilitation in Patients With a Laryngectomy: Proof‐of‐Concept of an Intratracheal Fixation Device

Abstract: Permanent hands-free speech with the use of an automatic speaking valve (ASV) is regarded as the optimal voice rehabilitation after total laryngectomy. Due to fixation problems, regular ASV use in patients with a laryngectomy is limited. We have developed an intratracheal fixation device (ITFD) composed of an intratracheal button augmented by hydrophilic foam around its shaft. This study evaluates the short-term effectiveness and experienced comfort of this ITFD during hands-free speech in 7 participants with … Show more

“…Over the past two decades, considerable efforts have been made to improve or develop new fixation devices to allow for adequate ASV fixation and promote regular hands-free speech in laryngectomized patients with a wide variation in stoma morphology, skin condition and preferences in the type of fixation device. 17,[26][27][28][29][30] The results of this study are promising for clinical practice. This study indicates that the moldable adhesive is a valuable addition to the wide range of fixation devices already available to laryngectomized patients, as it allows durable and airtight fixation during hands-free speech.…”

“…In some patients, the use of an intratracheal button or cannula in combination with an adhesive or the Provox ® LaryClip™ (Atos Medical AB, Malmö, Sweden) enables airtight fixation for hands-free speech. 17,26 To facilitate and improve ASV fixation without using additional fixation devices, a custom moldable peristomal adhesive was developed. When heated, this adhesive can be modeled by the patients to their stoma shape.…”

“…Новим етапом реабілітації голосу після ларингектомії стало впровадження хірургічних методів -трахеостравохідного (ТСШ) і трахеоглоткового шунтування [8,10,[17][18][19]. Одним з перших методику створення трахеоглоткового шунта розробив в 1965 р. R. Asai [14].…”
