In‐hospital acute stroke workflow in acute stroke – Systems‐based approaches

Abstract: The therapeutic goal for acute ischaemic stroke (AIS) patients is to achieve timely restoration of cerebral blood flow to the ischaemic brain tissue at risk of infarction and reduce neurological complications. 1 Current literature supports the efficacy and use of intravenous thrombolysis (IVT) with tissue plasminogen activator (tPA) and endovascular thrombectomy (EVT) in the treatment of AIS, with EVT proven to be more efficacious in patients presenting with large vessel occlusion (LVO). 2-5 When tPA and EVT a… Show more

“…Language barriers are associated with risks to patient safety and quality of care. They can pose a significant barrier in telemedicine uptake, specifically among culturally and linguistically diverse populations across low-resourced settings and developed regions alike, affecting providers and patients alike (9,(97)(98)(99)(100)(101)(102)(103)(104)(105)(106). Communication barriers have been shown to impact patients adversely.…”

“…Furthermore, the synergistic effect of in-hospital and prehospital interventions was not explored. 12 Moreover, means and standard deviations needed to be estimated, albeit with a verified method. Finally, some meta-analyses did not include many studies and participants.…”

“…Significant efforts have been put into optimizing the stroke workflow to minimize treatment delay in acute reperfusion therapy. 11,12 However, the prehospital setting remains a significant source of delay and an area in which workflow optimizations have been limited and where universal proven standards of care are lacking. Several studies on prehospital stroke workflow optimization (PSWO) have implemented varying strategies and shown success in reducing workflow time delay and patient treatment rates.…”

“…Due to cost-benefit and infrastructural reasons, as well as a shortage of neuro-interventionalists, not every spoke will have endovascular therapy capabilities. Additionally, barriers in access to health systems, applicable to culturally and linguistically diverse communities (CALD) as well as those from marginalized backgrounds or low resourced settings, merit special consideration (55,56).…”
