In vitro susceptibilities to amphotericin B, itraconazole, and miconazole of filamentous fungi isolated from patients with cystic fibrosis

Abstract: The antimicrobial activities of amphotericin B, itraconazole, and miconazole against 101 filamentous fungi from patients with cystic fibrosis were tested by a reproducible microdilution method. Itraconazole was very active against Aspergillus species and Scedosporium species (MIC at which 90% of the isolates were inhibited [MIC90], 0.06 to 0.5 mg/liter), whereas amphotericin B was less effective (MIC90, 0.5 to 8 mg/liter).

“…Equally, numerous cases of disseminated scedosporiosis with cerebral involvement have been reported after neardrowning, resulting from haematogenous spread from a primary pulmonary site of infection (Katragkou et al 2007). Additionally, S. apiospermum is a therapyrefractory pathogen since it is resistant or poorly susceptible to almost all current antifungals (Hennequin et al 1997). For all these reasons, S. apiospermum is a challenging pathogen and there is an urgent need for a better understanding of the pathogenesis of scedosporiosis.…”

“…Especially, severe and often fatal infections are described after lung transplantation, which constitutes the ultimate treatment of the patients (Symoens et al, 2006;Borghi et al, 2010;Morio et al, 2010;Hirschi et al, 2012;Miraldi et al, 2012). This fungus exhibits a low susceptibility to current systemic antifungal drugs (Hennequin et al, 1997;Alastruey-Izquierdo et al, 2007;Lackner et al, 2012). In this context, defining prevention measures aiming to reduce the exposure of the patients to the fungus is particularly challenging.…”
