Incidence and clinicopathologic correlation of fetal vessel thrombosis in mono- and dichorionic twin placentas

Abstract: Objective: To determine the incidence of fetal vessel thrombosis in monochorionic-diamniotic and dichorionic-diamniotic twin placentas, and its association with intrauterine growth retardation (IUGR), hypertensive disorders of pregnancy, twin-twin transfusion syndrome (TTTS), fetal vascular anastomoses, chorangiosis, and chorioamnionitis.Study Design: Histologic slides from 80 pairs of monochorionic and 80 pairs of dichorionic twin placentas were reviewed for evidence of fetal vessel thrombosis (X5 adjacent av… Show more

“…78,79 MC twins present also with a higher incidence of fetal vessels thrombosis compared to DC. 80 Fetal vascular thrombosis is found generally in association with IUGR in MC twins and with hypertensive disorders in DC twins. 80 The increased incidence of placental infarctions and thrombosis in DC twin placentas compared to MC and singletons may be associated with the higher prevalence of preeclampsia in DC twin pregnancies.…”

“…41 In addition to stillbirth, 42 perinatal liver disease, 43 and intrauterine growth restriction in monochorionic twins, 44 fetal vessel thrombosis has been linked to adverse neurologic outcomes, including neonatal encephalopathy 45 and neonatal stroke. 46 In the ELGAN cohort, it has been associated with ventriculomegaly of the offspring, 47 which, in turn, is associated with low MDI.…”

“…It is known that polycythemia in the recipient twin can cause hyperviscosity and hypercoagulation, producing erythrocyte aggregation that can cause vascular thrombosis [18]. The prevalence of fetal thrombotic vasculopathy in monochorionic placentas has been reported to be approximately 5.1-7.5% [15,19]. Further research is required to ascertain if fetal vessel thrombosis is more common in sTAPS, and whether this finding is correlated with perinatal outcomes.…”

“…However, TAPS can lead to perinatal death or neurological morbidities when managed via conservative measures [6,13]. Polycythemia may lead to vascular sludge, which may result in cerebral sinovenous thrombosis associated with intraventricular hemorrhage and parenchymal infarction, while fetal anemia may affect cerebral oxygenation and lead to hypoxic-ischemic cerebral injury [6,14,15]. Timed deliveries must also be weighed against the risks of prematurity [6].…”
