Incidence and determinants of sildenafil (dis)continuation: the Dutch cohort of sildenafil users

Abstract: Sildenafil utilization was prospectively evaluated among 153 men with a history of erectile dysfunction (ED)-prescription drug use prior to starting sildenafil and 164 men who were new starters of ED-prescription drugs. Further, some determinants of sildenafil discontinuation were identified. During a median follow-up period of 18 months 45% of all patients discontinued sildenafil treatment, regardless of earlier treatment history. However, patients with a history of drug treatment for ED were nearly eight tim… Show more

“…Marketing data worldwide showed that dropout rates for sildenafil are as high as 50% of the patients treated. 23 There are some limitations in this study; first, it was open label, second it takes place in only 12 weeks duration, and third we did not ask which pill the participants would rather take.…”

“…11 In a meta-analysis of 10 randomized, double-blind, placebo-controlled clinical trials, the discontinuation rate for patients prescribed sildenafil was only 10%, 12 compared with 35-45% for men with ED receiving sildenafil in a clinical practice setting. 13,14 The better treatment adherence rates observed in clinical trials may be due to better initial instructions on the use of the medication and to better follow-up with treatment.…”

“…DISCUSSION PDE-5 inhibitors are commonly prescribed as first-line treatment for ED; however, a significant proportion of men do not respond to PDE-5 inhibitor treatment. 14 The mechanism of action of PDE-5 inhibitors is through the inhibition of intracellular cyclic guanosine monophosphate (cGMP) degradation, but the underlying mechanism associated with non-responsiveness is unclear. Nevertheless, there are a considerable number of men who are PDE-5 non-responders and seek additional non-invasive treatment options.…”
