Incidence and Predictors of Mortality among Children on Anti-Retroviral Therapy in Public Health Facilities of Arba Minch Town, Gamo Gofa Zone, Southern Ethiopia; Retrospective Cohort Study

Abstract: Background: Evidence shows that earlier access to Anti-retroviral Therapy helps to increase survival of children by delaying the progression to AIDS. However its long-term effect on mortality has remained unanswered in Ethiopia especially in the study area.

“…Within a total follow-up time of 14,600 child-months, the mortality rate of HIV-positive children receiving ART in our study hospitals was 3.2 (95% CI: 2.3, 4.3) per 100 child-years of observation. Our finding is comparable with studies reported from Arba Minch Town Public Health Facilities, Gamo Gofa Zone, Southern Ethiopia (3.1 deaths per 100 child-years) [ 17 ], Nigeria (3.0 deaths per 100 child-years) [ 18 ], and Andhra Pradesh, India (3.0 deaths per 100 child-years) [ 19 ].…”

“…This study identified the predictors of mortality among HIV-positive children receiving ART using a multicenter facility-based historical cohort study. Within a total [17], Nigeria (3.0 deaths per 100 child-years) [18], and Andhra Pradesh, India (3.0 deaths per 100 child-years) [19]. Nonetheless, our finding is lower than studies conducted in Addis Ababa Public Hospitals, Ethiopia (12.4 deaths per 100 child-years) [20], Debre Tabor General Hospital and Dessie Referral Hospital, Ethiopia (6.3 deaths per 100 child-years) [21], and Kenya (8.4 deaths per 100 child-years) [22].…”

“…We found late initiation on to ART from time of HIV testing was predictive of reduced survival. Studies have shown that late presentation for early infant diagnosis (EID) services leads to poor prognosis (27,28). This is also evidenced by findings of Bolton-Moore et al (29) in earlier studies in Zambia.…”

“…Subgroup analysis was conducted by using the region and year of the primary studies published. From all studies which reported proportion of child mortality after ART initiation, 6 were from Addis, Ababa [ 18 , 26 – 29 , 33 ], 5 from Amhara [ 16 , 20 – 23 ], 2 from Oromia [ 30 , 32 ], 1 from Tigray [ 17 ], 1 from SNNPRS [ 19 ], and 1 from Harari [ 31 ]. In the subgroup analysis, the highest pooled proportion of mortality was reported in the Amhara region 16.76% (95% CI 9.63, 23.90) and the lowest in the Tigray 4.81% (95% CI 2.75, 6.87); subsequently, in Addis Ababa 10.43% (95% CI 7.29, 13.56), in Oromia 7.55% ( 95% CI 5.53, 9.58), in SNNPRS 15.40% (95% CI 11.95, 18.85), and in Harari 9.20% (95% CI 5.96, 12.44) were found (Fig.…”
