Incidence of endophthalmitis after cataract surgery in Japan

Abstract: . Purpose:  To estimate the incidence rate of endophthalmitis after cataract surgery performed by Japanese surgeons. Methods:  A sample of 20% of members of the Japanese Society of Ophthalmic Surgeons was randomly selected. Each member was sent a postal survey asking for information on the number of cataract surgeries performed in 2003 and the number of postsurgery cases of endophthalmitis. Results:  Replies were received from 78.7% (513/652) of survey recipients. The total number of cataract surgeries was 100… Show more

“…Methods used to derive contemporary figures for the endophthalmitis rate after cataract surgery include large-scale randomized trials, 14,15 single and multicenter studies in which a prospective reporting system is used to monitor changes in endophthalmitis rates over time, 9,16 and surveys of large samples. 13 All rely on accurate diagnosis and reporting of endophthalmitis cases. 12,17 Single-center studies and randomized trials may have more robust case reporting systems 14,18 but tend to have a smaller sample size and therefore wider confidence intervals around the rate (Figure 2).…”

“…The crude rate of 1 case per 6000 implantations is less than that reported in large-scale contemporary studies of endophthalmitis after cataract surgery ( Figure 2) . [9][10][11][12][13][14] However, caution is required in the interpretation. The methodology we describe requires participating surgeons to remember cases of endophthalmitis and to respond to a survey accurately.…”

“…26 In fact, this rate compares to that in cataract surgery with intracameral cefuroxime, as shown by the ESCRS study. 1 Nevertheless, when an intracameral antibiotic is needed, the drug chosen should be as effective and safe as possible.…”

“…Rabbits received gatifloxacin three times at a given interval of 2 h in that study, whilst the effect of dosing interval remained to be investigated. An in vivo animal model is a good alternative to such rare and severe disease [12][13][14][15]; however, animals potentially differ from humans in drug absorption and defence mechanisms against pathogens. The in vitro PK model, excluding defence mechanisms, would provide conservative information also suitable for patients with an impaired immune system.…”
