Incidence, risk factors and clinical course of thiopurine‐induced liver injury in patients with inflammatory bowel disease

Abstract: SUMMARYBackground: The incidence of thiopurine-induced hepatotoxicity in patients with inflammatory bowel disease varies in different studies. Aims: To assess the rate of thiopurine-induced liver toxicity in patients with inflammatory bowel disease; to determine the predictive factors and to characterize its clinical course and management. Methods: A cohort of 161 patients was prospectively followed for a median of 271 days. Hepatotoxicity was established when alanine transaminase or alkaline phosphatase plasm… Show more

“…17 In adult IBD patients, 4.6-10% cases were shown to develop thiopurine related hepatotoxicity. 18,19 Hepatotoxicity however, is not seen in all patients with high levels of 6MMP. This hepatotoxicity due to high 6MMP is especially problematic in AIH patients as raised transaminases can also be attributed to non-response or relapse of the disease.…”

“…Introduction We have previously demonstrated concerningly high 3-year mortality following hospitalisation with ulcerative colitis (UC) between 1998 and 2000 in Scotland. 1 We have extended these studies by examining 3-year mortality following hospitalisation with UC in Scotland between 2007-2009, providing an opportunity for comparison with our earlier results. Aim To compare 3-year mortality, and factors related to mortality, in Scottish patients hospitalised with ulcerative colitis (UC) between Period 1 (1998Period 1 ( -2000 and Period 2 (2007Period 2 ( -2009).…”

“…The occurrence of hepatotoxicity increases with concomitant use of corticosteroids [119]. Pancreatitis and veno-occlusive disease caused by Thiopurines is an example of the dose independent side event while hepatotoxicity is dose dependent (and hence is reversible with discontinuation).…”

“…Hepatotoxicity secondary to the use of Thiopurines ranges from 3% (in retrospective trials) to 10% (in prospective trials) [119]. The occurrence of hepatotoxicity increases with concomitant use of corticosteroids [119].…”
