Incidental Detection of Retiform Hemangioendothelioma by 68Ga DOTATOC PET/CT

Abstract: A 52-year-old man underwent Ga DOTATOC PET/CT scan for postsurgical staging of a neuroendocrine tumor of the lung. The PET images showed a single focus of increased activity in a soft tissue nodule located posteriorly to a cervical vertebra. Surgical removal of the lesion led to the histological diagnosis of retiform hemangioendothelioma.

“…5 Nevertheless, although 68 Ga-DOTATATE uptake has been described in many nonneuroendocrine tumors, no case of esophageal adenocarcinoma displaying 68 Ga-DOTATATE uptake has been reported before. [6][7][8][9] This particular case of metastatic esophageal adenocarcinoma showing 68 Ga-DOTATATE uptake highlights the potential of 68 Ga-DOTATATE PET/CT in imaging nonneuroendocrine tumors.…”

“…The presence of a significant number of somatostatin ( D ) cells in the columnar epithelium of the stomach and the association of Barrett esophagus as a precursor lesion in the majority of esophageal adenocarcinomas are considered to explain this SSTR expression 5 . Nevertheless, although 68 Ga-DOTATATE uptake has been described in many nonneuroendocrine tumors, no case of esophageal adenocarcinoma displaying 68 Ga-DOTATATE uptake has been reported before 6–9 . “…RH has also been detected by Ga-DOTA-tyr3-octreotide positron emission tomography–computed tomography. 13 Blood and urine tests are not yet specific. Lymph node ultrasonography is usually more valuable than sentinel lymph node biopsy, given that degenerative hemangioendothelioma rarely metastasizes.…”

“…Studies reporting incidental SSTR PET uptake in other locations were case studies (n = 7). Two cases proved to be malignant (29%), including retiform hemangioendothelioma in soft tissue and a non-Hodgkins lymphoma in a lymph node [15,27]. The remaining five benign cases revealed a pleomorphic adenoma in the parotid gland, parathyroid adenoma in close relation to the thyroid gland, elastofibroma dorsi in soft tissue, a uterine leiomyoma, and fibrous dysplasia in the femoral bone [8,[28][29][30][31].…”
