Incidental Lewy body disease: Clinical comparison to a control cohort

Abstract: Limited clinical information has been published on cases pathologically diagnosed with incidental Lewy body disease (ILBD). Standardized, longitudinal movement and cognitive data was collected on a cohort of subjects enrolled in the Sun Health Research Institute Brain and Body Donation Program. Of 277 autopsied subjects who had antemortem clinical evaluations within the previous 3 years, 76 did not have Parkinson’s disease, a related disorder, or dementia of which 15 (20%) had ILBD. Minor extrapyramidal signs … Show more

“…This may represent a significant confounding factor as the prevalence of incidental prodromal Alzheimer and Lewy body neurodegenerative pathology in cognitively normal elderly has been reported as high as 44% and 20%, respectively. Such presence of prodromal neurodegenerative pathology, frequently found in otherwise normal elderly subjects, may negatively affect olfaction (Adler et al 2010; Jansen et al 2015). Consequently, inclusion of non-screened elderly subjects with prodromal neurodegeneration in normative databases may artificially lower age-dependent hyposmia cutoffs.…”

“…The focus has also shifted to the analysis of patients without motor PD in whom LP are found at autopsy, i.e., incidental Lewy body disease (ILBD) [5,[16][17][18][19][20][21]. In this study, we report on the presence of LP in the autonomic nervous system of three PD cases and one ILBD case using alpha-synuclein immunohistochemistry in 100 μm thick tissue sections to visualize the morphology of LP.…”
