Incorporation of Donor Risk Into Liver Allocation Algorithms

Abstract: We would like to thank Avolio et al. (1) for the comments on our article on the Eurotransplant Donor Risk Index (ET-DRI) (2). The Authors address an important issue: the practical incorporation of donor risk into liver allocation algorithms. It is suggested that the ET-DRI should first be compared to existing donor-to-recipient-matching (D2Rm)-models, such as D-MELD (3), BAR (4) or SOFT (5), for applicability and accuracy before using it in liver allocation. We fully agree that transplantation outcome depends … Show more

“…Nevertheless, the D-MELD only incorporates one donor factor (age). The most interesting aspect about the ET-DRI (and DRI) is that it is the strongest risk indicator when looking at donor and transplant factors, and it is a continuous and validated score (8,27).…”

“…Numerous variables that may affect outcomes such as recipient age/gender/race, donor gender/race/liver steatosis, cold ischemia time, multiorgan transplantation, and pretransplant conditions are all excluded from MELD score calculation [35][36][37][38]. For D-MELD, an Italian study and further international correspondence appear to have clarified some of the drawbacks of the original study, such as its short follow-up period [39][40][41][42][43][44][45]. Therefore, the importance of appropriate donor and recipient matching cannot be overstated.…”

“…European Liver Transplant Registry data contributed to the validation of donor risk index (DRI) and balance of risk score (BAR) scores. Two papers looked at the DRI within the Eurotransplant area [49,50] as well at the definition of extended criteria donor (ECD) [51]. The DRI was markedly higher in 5723 patients belonging to the ET‐area compared to Organ Procurement and Transplantation Network (OPTN) indicating different donor populations.…”
