Increased numbers of cytokeratin-positive interstitial reticulum cells (CIRC) in reactive, inflammatory and neoplastic lymphadenopathies: hyperplasia or induced expression?

Abstract: A total of 291 enlarged lymph nodes showing a range of reactive-inflammatory processes, primary and metastatic neoplasms were studied to determine the distribution and immunoprofile of their cytokeratin-positive interstitial reticulum cells (CIRC) in comparison with normal nodes. In 258/291 nodes (89%), CIRC numbers were distinctly increased in the subcapsular, paracortical and, occasionally, in the medullary zones; often, these increased CIRC formed networks around follicles, sinuses and vessels. CIRC had com… Show more

“…Although neoplastic epithelial cells were not definitively identified in serial sections of lymph nodes with mucin-filled sinuses, epithelioid cells positive for cytokeratin-18 and negative for the macrophage marker F4/80 were present within the affected sinuses. Collectively, the presence of mucin lakes and cytokeratin-positive cells within lymph nodes are indicative of micrometastasis to the lymph nodes (29).…”

“…The range of antigen expression in FRC sarcoma has not been well defined. Nonneoplastic nodal FRC show uniform positivity for vimentin with subsets positive for actin, desmin, CD68 and keratin (32)(33)(34)(35)(36). However, the markers vimentin and CD68, also expressed in this case, are not lineage-specific and can be expressed in a variety of nodal sarcomas (see Table 1).…”

“…Such staining can easily be recognised morphologically. 29 Because no morphological checks can be made to control for false positive staining for cytokeratin in the MP-FCM assay, a threshold must be build into this assay. We found that when using a threshold of 1% in the MP-FCM assay a highly specific and sensitive distinction could be made between negative and positive SLNs.…”
