Increased risk of pemphigoid following scabies: a population‐based matched‐cohort study

Abstract: This study detected an increased risk for pemphigoid among patients suffering from scabies. Physicians treating elderly patients with a history of scabies should be alert to the development of pemphigoid.

“…The rate of BP was 0.3% in the first-ever stroke patients and 0.06% in control patients. The low incidence of BP in the control group is similar to that of the previous study [12] and the patients with neurological disorders [13]. Given the low incidence of BP, a large-scale cohort is needed to analyze the association of stroke and BP.…”

“…The autoantibodies generated in BP are specific to the transmembrane proteins (BP230 and BP180) and may not be completely responsible for the increased cytokines in atherosclerosis/endothelial dysfunction. As most BP is developed after the emergence of a neurodegenerative disease in the reported studies [2,7,9,10,12,13,14], our hypothesis may be more close to the clinical scenario in stroke patients.…”

“…The reasons for elderly predominance, we speculate, are that older individuals have increased likelihood of bedridden status and stay in nursing homes, resulting in increased risk of scabies infection. 52,53 The exact mechanisms are not fully understood yet. The lesions may involve, except the mucosa and face, the whole body including arms, legs, trunk, genitals, feet, buttocks, thighs, neck, inguinal folds, and may even be generalized; but the trunk and extremities are the most commonly involved areas.…”

“…[5] The literature also shows that BP may be triggered by infectious factors such as scabies in adult patients. [6] In addition, BP has also been shown to be associated with Coombs'-positive hemolytic anemia, membranous glomerulonephritis, ulcerative colitis, and hyperimmunoglobulin E syndrome and to develop after liver transplantation. [78]…”
