Indications, complications, and clinical outcomes of fixation and acute total hip arthroplasty for the treatment of acetabular fractures: A systematic review

Fortunato Giustra,
Giorgio Cacciola,
Francesco Pirato
et al.

Abstract: Purpose Acetabular fracture fixation can be challenging, especially in the elderly. Open reduction and internal fixation (ORIF) alone may not allow for early weight bearing and is associated with a high rate of secondary osteoarthritis; therefore, a combined hip procedure (CHP) or ORIF with acute total hip arthroplasty, may be beneficial in this population. The objective of this study was to perform a systematic review of all reported cases of CHP. Methods … Show more

“…On one side, there is general consensus on managing these fractures with ORIF in young and active patients. On the other one, no guidelines have been definitively established for the management of acetabular fracture in the elderly [ 2 ]. Indications for acute THA, i.e.…”

“…The most appropriate treatment for acetabular fractures is still under debate. Open reduction and internal fixation (ORIF) has been shown to have better clinical results, while THA allows for immediate weight bearing and has lower reoperation rate [ 2 ]. To maximize the outcome of acute THA, the patients should be carefully selected.…”

“…Автомобільні аварії з високою енергією часто пов'язані зі складними переломами та вивихами довгих кісток і таза. Зовнішні сили, що діють на великий вертлюг, передаються вздовж осі шийки стегнової кістки, викликаючи перелом вертлюжної западини, а продовження цих сил викликає одночасний перелом шийки стегнової кістки [4]. Ці травми вимагають швидкого та невідкладного лікування.…”

“…The combination of ORIF and acute THA is a complex surgical procedure that requires a mixed and advanced skillset from the orthopaedic surgeon[ 21 , 22 ]. The purpose of the ORIF in this clinical scenario is to restore the anatomy of the columns and thus create a stable bed for the subsequent implantation of the acetabular cup.…”
