We reported our initial experience of robotic‐assisted laparoscopic artery‐sparing varicocelectomy using indocyanine green (ICG) fluorescence angiography in treatment of varicocele. A total of 45 varicocelectomies in 27 patients were performed. The mean operation time was 49.1 ± 8.5 min for unilateral and 65.6 ± 8.3 min for bilateral repair. 47.2 s after ICG injection, testicular artery (TA) was visualised. After an interval of 31.3 s, fluorescent veins were identified. Of all the 45 spermatic cords, 68.9% had a solitary artery, while 31.1% had 2 arteries. The mean hospital stay was 1.6 ± 0.9 days. Semen concentration and motility were significantly improved 6 months after surgery, no recurrence, hydrocele or testicular atrophy was observed. Our study demonstrated that robotic‐assisted laparoscopic artery‐sparing varicocelectomy using ICG fluorescence angiography is a safe, effective and promising technique in treatment of varicocele.

The employment of fluorescence imaging has gained popularity in many fields of adult surgery, such as organ perfusion assessment, intra-operative solid tumor identification and sentinel lymph node mapping. In this respect, it has demonstrated great potential to improve both surgical and oncological outcomes while minimizing anesthetic time and lowering health-care costs.However, the clinical application of Fluorescence-Guided Surgery (FGS) in pediatrics is just at the initial phase.Review of published literature indicates a paucity of clinical studies evaluating FGS in pediatric surgery but huge scope for preclinical investigation and new clinical studies. Great emphasis has been given to the clinical application of FGS for surgical resection of hepatoblastoma and its metastasis and for real-time imaging of the biliary tree. Other current uses of fluorescent imaging in children concern the assessment of blood perfusion and lymphatic flow with the aim to prevent iatrogenic injuries during intestinal and urogenital surgeries. While new and more specific probes are under development, the only fluorescent dyes safely employed in children are currently indocyanine green and fluorescein sodium. Advantages, limitations and potential future development of FGS in pediatrics are systematically discussed and analyzed.

Over the past few decades, there have been numerous paradigm shifts in male reproductive surgery, resulting from a combination of technologic advancements in both the operating room and the laboratory. The operating microscope transformed the field of male reproductive surgery, enabling novel techniques and higher precision for the treatment of male-factor subfertility. The subsequent widespread adoption of microsurgical approaches was largely responsible for the emergence of a cadre of highly specialized male infertility microsurgeons. The advent and evolution of in vitro fertilization/intracytoplasmic sperm injection was a concurrent story that further revolutionized the field. The ability to achieve fertilization and pregnancy with just a single sperm changed the ways in which male reproductive surgeons could approach a wide range of pathologies from obstructive to nonobstructive causes, culminating in the microdissection testicular sperm extraction procedure for the treatment of nonobstructive azoospermia. Here we review the recent advancements in fertility-enhancing male reproductive surgery for the treatment of four disease processes: varicocele, obstruction of the excurrent ductal system, ejaculatory duct obstruction, and nonobstructive azoospermia.
