IntroductionThis study, of significant importance to healthcare professionals, policymakers, researchers, and organizations involved in child healthcare and malnutrition in Afghanistan, aimed to estimate the out‐of‐pocket expenditure (OOPE) in patients under 5 years old with severe malnutrition in a children's hospital in Herat Province, Afghanistan.MethodThis study employed a meticulously designed cross‐sectional descriptive‐analytical approach with practical results. The research population consisted of families with malnourished children under 5 who were referred to Herat Children's Hospital. Data was collected using a comprehensive standard World Health Organization questionnaire to gather demographic information from children in Herat. A carefully selected convenience sampling method was used, with 300 referring patients participating in face‐to‐face interviews with the supervisors of these children. After obtaining personal consent and coordinating with health officials, interviews were conducted with the caregivers of children under 5 who suffered from severe malnutrition. The data was then analyzed using robust descriptive statistics, quantitative variables, mean and standard deviation, frequency, and relative frequency. Multiple regression analysis was used to determine the factors that most influenced direct payments from patients' pockets, ensuring the reliability and validity of the findings.ResultsThe results showed that OOPE in both households with seven and less than seven people and more than seven people was 68%. The findings indicated that among the residents of Herat referred to the studied hospital, these people spent 54% of the treatment costs directly out of pocket. In contrast, people in the rural areas of Herat pay 69% of the treatment costs to receive medical services straight out of pocket. The critical point is that 93% of the families have incurred catastrophic expenses to treat their children suffering from severe malnutrition. The research revealed that the patient's location and the education level of the head of the household were the most significant factors affecting out‐of‐pocket payments by patients.ConclusionIncreasing OOPE in rural Afghanistan poses a significant obstacle to equitable healthcare services and access to appropriate medicines. To support the goal of universal healthcare coverage, geographic imbalances, and broad health financing options must be addressed. Strengthening insurance coverage and more government assistance can significantly reduce these patients' out‐of‐pocket payments.

Purpose This study aims to formulate a consensus on primary care physicians’ leadership competencies tailored to Indonesia’s rural and remote health systems using the LEADS framework. Effective physician leaders are essential in these settings; however, many physicians lack the necessary leadership qualifications due to insufficient training. From a medical education perspective, this issue is further compounded by the lack of consensus on leadership course content. Design/methodology/approach The study used a mixed-method approach with a concurrent triangulation strategy. Concurrently, a two-round Delphi study and qualitative interviews were conducted. The Delphi study involved academics, rural and remote primary care physicians, intern doctors (similar to house officers), clerks and medical students and used descriptive analysis. Semi-structured interviews, guided by an interview guide, were analysed using inductive thematic analysis. Findings From the initial 62 LEADS framework statements, the authors identified 52 physician leadership attributes in round 1 and 41 attributes in round 2. Qualitative interviews revealed three main themes: the significance of physician leadership, the physician leadership curriculum and its potential and impact. Originality/value The study established consensus and provided scholarly insights into the leadership development necessary for primary care physicians in rural and remote areas. This is essential for developing Indonesia’s medical leadership curriculum, with the ultimate goal of improving health outcomes in these settings.

This study aims to evaluate the durability of as-cast Ti-6Al-7Nb in femoral stem application using fatigue analysis. As-cast Ti-6Al-7Nb rod specimens were tested experimentally with load range of 120%, 100%, 90%, 80%, 70%, 50%, 30%, and 20% relative to the yield strength. The fatigue limit was identified on a range of 141 - 211 MPa, represented by two run-out specimens running until high fatigue cycle (HFC) of 107 without failure. ProFatigue contributed to expose the estimation of the expected failure cycle on fatigue limit stress of as- cast Ti-6Al-7Nb based on Weibull modelling. It was predicted by probability failure (Pfail) of 50%. The failure cycles were revealed at 2.144 x 107 and 2.823 x 109 cycles on amplitude stress of 211 and 141 MPa, respectively. The reliability of the designed femoral stem for Indonesian people has been analyzed under ISO 7206 configuration through finite element analysis by adopting the material properties of as-cast Ti-6Al-7Nb. The static linear analysis demonstrated the maximum stress concentration of 136 MPa occurring over the medial region of the femoral stem, showing lower stress levels compared to the fatigue limit of as-cast Ti-6Al-7Nb. Furthermore, the lifetime analysis computed that the design could withstand cyclic loading until 2.51 x 109 cycles with fatigue safety factor and damage factor in 1.763 and 0.398, respectively. Results concluded that the investment casting is evident to become an alternative manufacturing method of femoral stem made of Ti-6Al-7Nb.
