Indwelling Tunneled Pleural Catheters for Refractory Hepatic Hydrothorax in Patients With Cirrhosis

Abstract: We present, to our knowledge, the first multicenter study examining outcomes related to IPC use in HH. Ten percent infection risk and 2.5% mortality were identified. IPC placement may be a reasonable clinical option for patients with refractory HH, but it is associated with significant adverse events in this morbid population.

“…[82][83][84] Chen and colleagues showed a 16.7% risk of pleural space infection that was confirmed in subsequent studies. [85][86][87] VATS to repair the diaphragmatic defects is another potential solution, but again studies are small, although results were impressive with 70 to 100% success rates. 84 However, many of these patients may be critically ill and malnourished, making the operation a high-risk one.…”

“…84 However, many of these patients may be critically ill and malnourished, making the operation a high-risk one. IPC has been the latest development in the treatment of refractory HH, but complications remain high at almost 40% 86,87 and autopleurodesis rates remain low. 87 Heart failure remains the most common cause of pleural effusions.…”
