Infection of sand flies by humans coinfected with Leishmania infantum and human immunodeficiency virus.

Abstract: Abstract. To determine the role that Leishmania infantum/human immunodeficiency virus (HIV) coinfected patients could play in the epidemiology of visceral leishmaniasis (VL), we applied direct xenodiagnosis of VL in this study to test the infectivity of six coinfected patients to colonized Phlebotomus perniciosus. All patients proved to be infective for the sand flies. The infectivity of patients who had still not received specific treatment for VL was inversely proportional to their absolute CD4ϩ T lymphocyte… Show more

“…The infectiousness of dogs, with and without symptoms, has been measured in at least 13 reported studies (Courtney O, unpublished data) with a mean infection rate of 25% (1,979 of 7,927 sandflies). 7,27,28 In contrast, only 8% (112 of 1,365) of sandflies fed on VL patients were infected, 8,18,29,30 and none of about 2,000 sandflies fed on subclinical cases or cured VL patients became infected. 18 Previous attempts to test the role of dogs focused on household dog ownership patterns.…”

“…This fact may explain the high rate of infection due to the whole spectrum of immune response to Leishmania in the canine population (4). In human beings, a strong association between the decrease in the number of CD4 ϩ T lymphocytes and the increase in the infectivity of people coinfected with L. infantum and human immunodeficiency virus to the sandflies has been reported recently (16).…”

“…In addition to the identification of other reservoirs 9,10 , some authors have raised the possibility of humans themselves serving as a reservoir for the disease, principally in relation to leishmaniasis/HIV co-infection 11 . However, the impact of these findings on transmission of the disease in the urban context is still unknown.…”
