Inferior Pancreaticoduodenal Artery Aneurysms in Association with Celiac Axis Stenosis or Occlusion

Abstract: These aneurysms are thought to arise due to increased flow through the pancreaticoduodenal arcades. The arcades first enlarge, and then form focal aneurysms which may rupture. The aneurysms can be treated through endovascular techniques or by surgery, though the former is a preferred approach.

“…4 Almost half of true PDA aneurysms are associated with stenosis or occlusion of the CA. 4 Sutton and Lawton first suggested that CA occlusion or stenosis was the underlying cause of true PDA aneurysms, 5 as stenosis of the CA increases blood flow in the peri-pancreatic arterial network and provides collateral vessels for revascularization of the CA. The chronic increase in blood flow through the PDA weakens and dilates the arterial wall, which, in turn leads to the formation of an aneurysm.…”

“…Aneurysm can be associated with the development of abundant collateral circulation. 2,3 The aneurysm itself is asymptomatic at first, then with nonspecific abdominal symptoms, and may lead to rupture and potentially life-threatening intraabdominal hemorrhage. Less asymptomatic aneurysm inci dentally detected by CT examination.…”

“…Therefore, it is necessary to treat all types of diagnosed aneurisms. 2,3 When conventional selective angiography or CT angiography diagnoses visceral artery aneurysms, it is necessery to preform a dynamic evaluation of collateral blood flow and diagnose the possible obstruction of the main splanhnich artery. CT angiography isa more recent and very successful non-invasive method to achieve an accurate diagnosis.…”

“…However, their association with celiac artery compression or occlusion has been well described. 5, [17][18][19][20][21][22][23][24][25] Pseudoaneurysms are more common than true aneurysms and are caused by pancreatitis, infection, and less commonly trauma. [26][27][28][29] Visceral aneurysms associated with polyarteritis are rare and can be lethal.…”
