Inferior Vena Cava Leiomyosarcoma: What Method of Reconstruction for Which Type of Resection?

Abstract: Inferior vena cava leiomyosarcoma (IVCL) is a rare tumor with a poor prognosis, and its surgical resection remains a challenge. To date, surgery is the only potentially curative treatment for IVCL with a 5‐year survival rate of 55%. The main challenge is to combine oncological surgery with clear margins and vascular reconstruction of the inferior vena cava (IVC). In this review, we discuss the different approaches to vascular reconstruction after IVCL resection, using a prosthetic or autologous patch, direct s… Show more

“…IVC reconstruction is not always indicated. It depends on the presence of a good network of collaterals, ensuring venous return [16,18]. Several techniques have been described for the IVC reconstruction, and various materials have been used, like synthetic materials including Dacron, polytetrafluoroethylene, vascular graft, or banked vena cava homograft tubularized bovine pericardium, and a stapled peritoneal fascial graft [7].…”

“…There were several options for IVC repair, including ligation, primary repair and IVC reconstruction. [6, [15][16][17] According to Yoshidome et al, it was necessary to reconstruct vena cava only in patients with poor collateral circulation or unstable hemodynamics. [17] On the contrary, Illuminati et al suggested vascular reconstruction if applicable.…”

“…[15] In our center, we were in favor of reconstruction via graft when there was bad collateral circulation in the upper and/or middle segments of IVC. [4,15] For the lower segment of IVC and middle segment of IVC with enough collateral circulation, we preferred primary repair. Compared to ligation, the IVC after primary repair was patent at least to some extent, providing an out ow for venous drainage.…”

“…Furthermore, the management after the surgery resection of tumors is also vital. It is still controversial for the need and different strategies used for inferior vena cava reconstruction, which depends on the tumor location and hepatic or renal vein involvement ( 4 ). However, in recent years, there are few clinical studies and reference experience on leiomyoma.…”
